It's a worthless idea. Are you sure someone will need your organs after your death? Or that someone will find your dissent and quickly present it to the doctors?

I don't think there's a single database. And the lack of organs can't be verified by relatives.

I don't know how old you are, but if your age is respectable, no one will need your organs if you are young. You can write a notarized statement and let your relatives keep it. No one of my friends, even in the military, has written such a statement.

Better during life and notarized. After death, the relatives won't have much time. On the other hand, why would the deceased need organs? You can save someone.
